Southwest Regional Medical Center to hold Multiphasic Blood Screening on November 23September 25, 2013
Southwest Regional Medical Center, in conjunction with the Rotary Club of Waynesburg, will once again conduct the Multiphasic Screening on Saturday, November 23, 2013, from 6:30 am – 9:30 am. The screening will be held on the second floor of Southwest Regional Medical Center in the Guesman Room. Breakfast will be held in the Southwest Regional Medical Center café following the screening.
All tests offered through the multiphasic screening will be completed in the Laboratory of Southwest Regional Medical Center. In an effort to remain 100% compliant with current State requirements in administering the Multiphasic Blood Analysis program, patients who wish to participate in the screening will need to do the following:
- Have your physician sign the attached form indicating his/her willingness to accept the results of your tests. (A copy of your results will be mailed to your physician within approximately 15 days after the screening date.) Also, if you are interested in receiving a copy of your tests, please have your physician mark that he/she permits the hospital to mail a copy of the results to you directly. The State requires physicians to approve results being sent directly to their patients. PLEASE NOTE: A copy of your results mailed to your home is optional. If you choose to have a copy mailed directly to you there is an additional charge of $5.00.
- After receiving your physician’s signature to accept your results, complete the attached form indicating which tests you would like to receive, your appointment time choice, and your information. Once the attached form has been completed mail the form with payment to:

- Upon receiving your form, Southwest Regional Medical Center will send you a confirmation letter indicating the time of your appointment, which tests you are scheduled for and the amount paid. If you send your information past the deadline, you may not receive a letter as time may not permit us to send it in time. However, you can assume that your time preference will be honored unless you hear otherwise.
Southwest Regional Medical Center must strictly adhere to the above procedure in order to be compliant with State requirements. No exceptions will be made.
